i think its neither, the institution was jus playin him and he didnt fall for it at the end.

why u ask?

1. just cos theres a storm dont mean ppl become sick.. he had a headacke when they gave him water.. they drugged him. so he ended up having migrains etc bad dreams..

2. the nod at the end was for him to leave, the 2 guards stepped a side and he walked off to go home, not to the tower.

however, the begining of the film, it could be that the guards r uneasy cos they know leo, is the real thread, the biggest in the island, which explains the tension at the start there..

truthly i dont no, im still tryina figure this shit out lmao.

nice theories lol ^^^ SPOILER ALERT FOR THOSE WHO HAVEN'T SEEN SHUTTER ISLAND

I saw the movie again. It's crazy how many clues there are throughout the movie. Here's what I think about the ending.

1)He really is insane and the radical role play set up by the doctor did not stop the "tape from rewinding."

2)At the end of the movie, Leo's character says "I wonder what's worse, to live a monster or die a hero." He then walks away with the orderlies. I think he knows he's Andrew Laeddis but he'd rather die as Teddy Daniels, the hero, than live as Andrew, the monster. The "drastic measures" they said they would take if the role play didnt work would be either killing him, or taking him to the lighthouse, IMO. Either way, I think Leo's character would rather die/go to the lighthouse as Teddy than live with the guilt of killing his wife and kids.
